ACOVE

ACOVE is Assessing Care of Vulnerable Elders, a joint RAND/UCLA project, that is designed to measure and improve quality of care for the vulnerable elderly—those most likely to die or become severely disabled in the next two years. In 2007, ACOVE-3 updated quality indicators (QIs) for 26 conditions. The final set contains 392 QIs covering 14 different types of care processes (e.g., taking a medical history, performing a physical examination) and all four domains of care: screening and prevention, diagnosis, treatment, and follow-up and continuity. All QIs also apply to community-dwelling patients aged 75 and older.

ACOVE-2 uses practice redesign approaches (screening, structured visit notes, delegation to office staff, linkages to community resources) to improve quality of care for specific geriatric conditions by physicians or nurse practitioners. In clinical trials the ACOVE-2 intervention has led to improvements in quality of care provided for falls, urinary incontinence, dementia/cognitive impairment, depression, and heart failure.

WISE & Healthy Aging Physician Liaison Program

We work with the medical community to help physicians care for their older patients. When requested, we intervene with psychosocial assessment, and provide linkages to in-home and community services for older adults.
