Team Care to Manage Chronic Illnesses in Older Persons

This project will implement and evaluate a nurse practitioner (or physician assistant) co-management intervention to improve the care of 5 geriatric conditions (falls, incontinence, dementia, heart failure, and depression) in 3 small (5 or fewer physicians) group primary care practices. The recruited practices will serve communities in the 4 areas that UniHealth has identified as priorities. The proposed program changes the basic structure of care to create a physician-nurse practitioner (or physician assistant) team to increase the effectiveness and efficiency of office practice. The nurse practitioner or physician assistant is a true partner in providing care and in this role enables the small group practitioner to deliver the highest possible quality care. By creating a model that improves quality of care and can be easily assimilated into current practice, the project will have excellent potential for widespread dissemination.
